Hi Becky,
I too, an a hospital LC working primarily with term and near term infants.  Our NICU has been using LOTS of banked donor milk for the premies for the last 3-4 years.  We start weaning them off donor milk when they hit 34 weeks corrected gestational age.  Like you, I was really keen to make donor milk available to the term babies who need short-term supplementation for medical reasons.  I contacted HMBANA, wrote to Mary Rose Tully and several others who have done a lot of work on banked milk.  What I learned is very few if any hospitals are using banked donor milk for term kids.  So you cannot make a case by going to your management and saying, the industry standard is to make banked milk available.  

I met with our nurse manager about this, and we calculated the annual costs would be (if I remember right) about $4000, for a population of 3000 deliveries a year, with a bfdg initiation rate in the high 80%.  We have discussed having the pharmacy order it and charging it out as a  medication.  I was told that will not work administratively b/c banked milk does not have a pharmacopoeia number, and therefore cannot be charged for by pharmacy.  

We have had a couple families who REALLY wanted it, and what we came up with was having the mom place an order from the milk bank using her own credit card.  The milk bank ships it out Fed-Ex, I think same day if you order before 10AM, and the milk comes next day.  Our closest bank (San Jose, CA) has a minimum order of 50oz, so it costs the family $200-$300.  For the moms who have done this, we borrow banked milk from our NICU freezer stash, and replace it when the mom's order comes next day.  If anyone has a better idea about how to make this happen for term babies PLEASE SEND IT TO ME!!

I would like to hear from people at hospitals who are making donor milk
(from a milk bank) available as an alternative to formula when there is
a medical indication for supplementation.  I'm not talking about NICU
babies, just for term and near-term infants.  We initiate pumping within
the first 12 hours if a baby isn't nursing, but it seems many mothers
get absolutely nothing or just drops of milk for several days.
Sometimes that's okay, and sometimes it isn't.  If your hospital is
making this available, how many of your parents choose pasteurized donor
milk over formula?  And do you have a policy or consent form you could
share?  I did look at the CPQCC documents that Dr. Wight has referenced,
but only found consents for fresh donor milk, not pasteurized.  If I
missed it, let me know.  Thanks, all.
