I just had a moment to recall this book.  I have it.  It's from Childbirth
Graphics and it's called "Breastfeeding Triage Book".  I just looked at the
inside cover and it is indeed from Seattle-King County Dept. of Public
Health.  I agree that it's not totally conclusive for a complete phone
assessment, but it's concise and it can give you tidbits to use, or perhaps
trigger some ideas you may not have thought about.  I actually haven't used
it that much.  One example listed:  Baby Not Gaining Well.  Then it lists
possible problems in different sections.  One problem is: Hungry Baby.  The
book then clearly articulates that this book is intended for a phone
assessment, and that further evaluation by an LC is necessary.  Then possible
causes with related plans are listed.  On the back of the book, the contact
