<Nikki raises a good question. This whole thing about diagnosing bothers me a lot, because truth be told, there are a lot of things that we are better at diagnosing in the lactation world than many HCPs are. In the nursing world you have "nursing diagnosis" and in the lactation world I believe in "lactation diagnosis."  >

And in today's world, insurance companies pretty much require a diagnosis, and the organization for which I work requires a diagnosis for every visit.  The solution we came up with where I work was to consult with the managers and physicians, and agree on a couple of diagnoses we would use.

And how do I use a SOAP note (which is common where i work), and avoid diagnosis?  Somehow write an assessment that gives a great description while skirting around using particular terms?
