Sorry I didn't answer sooner but got tied up in preparing a Talk for the
forum next week here on Safe Motherhood in Australia.

Well, In Northern Territory for last 4 years-
At the Darwin Private hospital the Hospital stickers(Hospital No., Mother's
Name and time and date of expressing) are used to label the small jars. and
they are refigerated or frozen. On use in the Baby Fluid chart the sticker
of that jar was taped onto the back of the Fluid chart and on each feed the
checking was done by two staff members initialling as checked with the baby
name tag against the mother's number and name.

At Royal Darwin, a Bigger unit in NICU and Special Care Nursery. Both use
the Mothers Name/ Hospital Number Sticker for the Expressed Breast Milk then
on use (whether refrigerated or frozen, Removed from a Special Fridge/
freezer) the EBM is measured by Syringe into a sterile bottle for use, the
label being checked and signed for by two nurses on the baby Fluid Balance
Chart in a special column.

In Kalgoorlie Western Australia for 10 months in middle of the above. It was
a relatively small unit but all EBM was checked with label on baby and
Mother's label and the Labels were put on the Back of the Baby Fluid Chart
after initaling the Drug Chart where the Bresast Milk was ordered.

While in Wodonga, Victoria, where I workrd pre 1996 we checked all formula
with Mother when she brought it in or Expressed at the unit. Mum and the
Nurse signed and dated the label that had mothers and Baby's name and Baby's
number.
It was then refrigerated or if frozen stored in the freezer. This wasn't
always possible as sometimes it came in, in an esky(insulated container)
frozen from a distant town by Greyhound bus, as -'Urgent-special delivery'
Mother's name and time dated of each container of each and it would go
straight into the Freezer stored as Freshest date to the back and earliest
date in front. When needed it was thawed and syringe measured and bottled as
required for feeds. This was checked by two staff as it was poured and the
Baby Fluid chart Initaled as so.

Here in Quetta, I express Mum and feed fresh as there is no Refrigeration
Boiling Syringes and container to express into pre feed.
