I first saw it described nicely in the book, Nursing Mother's Companion.
Huggins showed the different angle needed for cradle vs. football.  Also
liked Diane Wiessinger's article in JHL on positioning awhile back.

I don't believe the tea cup hold described is the same as the sandwich though
the pinched portion would be to make a sandwich of sorts in the angle similar
to the description.  I suppose though that the terminology for these
different techniques has probably gotten mixed around among us.  I think of
the tea cup hold as more of a tissue gathering together technique that makes
a nipple out of one that isn't prominent.
