Sounds like possible oversupply/overactive letdown.

Baby is breaking suction. I have seen several babies do this to handle an onslought of milk either by oversupply or by an overactive letdown. There are other clues that also point to this issue, such as the reflux and obviously some gut issues as well since mom is on a milk & soy free diet. Another clue is that it better if she pumps an ounce before feeding him. That is where I would start if there are no obvious structural issues with baby. If she has oversupply/overactive letdown, initiating a block feeding regimen may downregulate her supply enough to make things a lot easier for both of them.
