We had this problem with Anna, too.  That diet of breastmilk, bananas, rice,
and avocado did nothing for her bowels.  Our pediatrician told us to feed
her "P" foods.  Peas, peaches, pears, prunes, etc.  I'd mix half a jar of
prunes with breastmilk and some cereal (oats were better for us than rice)
once a day, or substitute pears or peaches for the prunes.  We were only
dealing with one solid meal a day at that stage so every few days we'd try
pureed peas instead of the fruit and cereal meal.  She got back on track and
hasn't had a single issue since (she's almost 5)!
