The last 2 babies we saw through our outpatient department that were refusing to breastfeed had raging ear infections. Both babies, according to the moms, were not symptomatic, but my co-worker noticed they were waving their fist and batting at/near their ear! 

Something to consider if the ped hasn't already ruled it out.
