Tara - have you considered Reynaud's? Sometimes I've seen this start  
when there's an initial trauma or infection to sensitize the nipples -  
and, fluconazole (Diflucan) is also a vasoconstrictor, so can actually  
make symptoms of Reynaud's worse. Ask mom about any color changes in  
the nipple after feeds. Just about any stimulus can start an episode  
once the Reynaud's is started; stress, air conditioning, brushing of  
the nipples, etc. There are many treatments for Reynaud's, but many  
moms get increased comfort just keeping the nipples out of drafts and  
warm right after feeding.
We have brutal thrush here in moist and humid Vermont, and see a fair  
bit of Reynaud's associated.
