We are in the process of revising our hospital's outpatient lactation visit
documentation form.  (Yes, I know there's computer software programs out
there, but for right now, we're not able to purchase one.)

So, would anyone be willing to share their outpatient lactation
assessment/documentation form?  You could send via fax or snail mail.  If
pdf form, could email if that's easier.    Address/fax below.
