This mother is likely to be registered with a GP (General
Practitioner, or family doctor) and a health visitor.

If she has a GP and the GP won't treat, she can ask to see another
doctor in the same practice and discuss it with him/her.

She can also ask her health visitor to discuss it with her GP - her
HV may not be able to prescribe, but she can advocate for and with
the mother.

We don't have MDs in the UK :) Well, we do, but MD is not the same as
MD in the US.

However, ductal infection may not be accepted as a diagnosis, and
even less likely as a solely self-diagnosis....but she can at least
raise it with her HCPs.

Hope she gets support with her problem.
