Hi.. I will quickly re-introduce: I am a nursing mom, LLL Leader, and CLC. Got a call today from a mom who gave permission to post. Mom nursed previous baby 11 months, had frequent mastits & surgery for breast abcess w/ 1st baby. Clean mammogram a year ago. This time, milk came in at 24 hrs post partum, had lots of engorgement for the first 6-8 weeks. Baby (11 weeks old) is a "snacker". For several weks now mom has experienced extreme pain in right breast, describes it as stabbing and constantly achy/throbbing. The baby vomits blood-streaked milk after feeding on that side. Mom describes the milk as looking like it has clotted & been "painted with pink streaks" when baby vomits. There is no visible bleeding & milk looks normal when she pumps. She had pain & swelling in underarm area as well; that has reduced but left a lump. Pain is in the left side of and under the breast. The left breast is not painful, but is visibly smaller than the right and mom says it produces less milk, so she is concerned about baby only nursing on that side. Since December has had "shooting pain" in the right breast; has been treated with loading dose of Diflucan but no full course 2x. She had the yeast symptoms for quite awhile before seeking treatment. She is currently treating herself & baby with Nystatin. Doctor has told her that she needs to locate the duct where the blood is coming from so they can do a "ductogram" and to pump & freeze the milk until they have been able to do that procedure vs. continuing to nurse on that side. I told her to talk to the doctor about a longer course of Diflucan to clear the yeast, pump OFTEN to keep supply up on that side & to avoid adding mastitis to what is already happening, and consider a specialist evaluation re the bleeding. ANY input or advice welcome!! Thanks!!!
