Further thoughts generated by further posts.  I have never seen a baby taken
off brestmilk because of jaundice at my hospital, although one still hears of
it.  Healthy, full-term babies beyond the first two days are only re-admitted
for phototherapy if the bili is approaching 20.  Mom stays in the hospital if
possible and frequent breastfeeding is encouraged.

Glucose testing is another question.  Any SGA or LGA baby (and LGA is still
defined as 4000gms for a term baby!) gets a pretty intensive monitoring
protocol.  The other day, parents of a baby just barely over the line, maybe
4030, were getting tired of seeing their baby get poked and expressed their
concern.  I said, "Well, I think he's fine, but this is our protocol.  If you
refuse to let me continue, I have to honor your request and will chart it in
my notes."  They happily state their refusal and I happily charted "Parents
refuse further glucose testing for baby" and that was that.  Of course, I also
