Lisa and Holly,

Sulpiride has potential pyramidal side effects, and also maternal weight
gain, and so isn't frecommended in m ost coungtrie. See p.685 of the 3rd
edition of the Core Curriculum for Lactation Consultant Practice (Jones &
Bartlett, 2013) in my chapter on induced lactation and relactation. Similar
comments ts were made in the 2011 revision of the Academy of Breastfeeding
Medicine's Protocol # 9, on galactogogues. When I last looked, this
protocol as been taken down for review this year. The ABM emphases trying
the normal ways of improving lactation, first.

The key message, as you will be aware, is that other methods of
establishing and increasing milk yield should be used before the mother
considers turning to chemicals (pharmaceutical or botanical). When a
particular substance is very popularly used as a galactogogue in a culture,
the upshot is that it becomes the first step and not enough attention is
given to proven strategies - such as increased milk removal (by direct
suckling or hand or machine expression), a better latch, attention to
underlying infant or maternal conditions. "Everyone" wants a "quick fix" -
which this isn't.
