I had nothing to do with the protocol you are referring to, but I read that section as speaking to a baby who is NOT gaining well.  If they are NOT gaining well, (despite good breastfeeding technique including breast compressions) then they should not spend more than 30 minutes at the breast (because they are actually sleeping more than eating). AND feedings should be more frequent  (If they are not currently frequent enough) AND mom should pump AND baby should be supplemented with expressed milk.

I agree that a baby who is doing well may spend more time at the breast than 20-30 minutes, and that would be fine assuming they are gaining well. It would also not be appropriate to limit time at breast WITHOUT the pumping and supplementing part.
