Hi fellow Lactnetters from hot and humid central Iowa.  I am trying to
develop a survey to give to the moms we consult with in order to get some
feedback on the Lactation Services offered at our hospital.  I work in a
hospital that has over 3,400 birth per year with a NICU that serves up to
50 babies.  We are looking for several short questions that would help us
evaluate our services, keeping in mind that there is only 1 1/2 full time
L.C.s on staff trying to do the job as well as possible. If you have
developed one or know of how I could get some ideas for starting one,
please let me know.  The heat has melted my brain, of course.  I really
appreciate any help you could give me.
