Pam, In our large hospital we get quite a few  multiple births.  We usually
have the mothers start to pump as soon after birth as they are able if the
infants are not yet able to go to breast.  We advice them to pump at least 8
times a day for 10-15 minutes, sleep during the night if desired but get up
once at night once the milk comes in.  We sometimes have difficulty getting
the mothers that have had a C-section to pump early if they haven't discussed
breastfeeding during the pregnancy or if  well meaning friends and family
tell them they should get their sleep while they can.  We also do seem to run
into a problem with mothers of  the really early infants trying to maintain a
supply (28weeks)  In some cases it is because the mothers are pumping but
having a hard time pumping 8-10 times a day long term when they may be only
getting small amounts.  They need praise and encouragement for the great job
they are doing and the great time that it takes to do it.  They need to
realize that they only have  to keep up with the infants need; they don't
have to get great amounts because hopefully as the infant matures he will
start to breastfeed more and more effectively and bring in an increased milk
supply.
