The mom has had the blebs "roto-rootered" by the MD and then has been able
to clean them out herself on occasion?

The fact that mom feels "slightly better" when she uses lansinoh makes me
wonder if the lansinoh is retarding a reparative process gone awry: skin
overgrowth that is trying to heal something. 

Your plan of having mom put polysporin on her nipples is where I would have
started also. But I wonder if Jack's 3-in-one ointment might be an even
better thing to try? Or the polysporin or Jack's ointment mixed with
lansinoh? A little bit of steroid cream might help reduce any inflammation
that could contribute to the chronic reaction.

Here's another off-the-wall thought: hydrogel type pads. If mom's body is
trying to repair something, both lansinoh and those pads are supposed to aid
healing and minimize the usual scab-skin process. Could try one, the other,
or both. 

I also wonder if daily (or 2 or 3x) saline soaks might be of benefit.
Whether there are solid plugs, skin-covered blockages, or both, frequent
soaks may help to keep everything softened and moving. They are best done
prior to feeds.
