This sounds like vasospasm.

Have her try a heating pad on low steeing for at least 5 mins after EVERY
nursing or pumping until there is no more pain. (often in as little as 48
hours)  You must ensure that nipple trauma has stopped...ie baby pinching to
slow flow, pump flanges that don't fit, bad pump with too much suction,
yeast.

If she did not respond to GV or Diflucan, this is not yeast (in my opinion).
And if she did, vasospasm often is triggered by the inflammation associated
with tissue damage from any cause.
