Tangentially related:

One of our staff referred a mom she was seeing to an LC for more expert
followup.  The family physician (who do most of the well adult and
infant care health) reamed out the nurse and complained to the manager.

Now keep in mind we are talking about public health nurses.
Periodically we get "orders" from a physician.  We laugh at them,
because we operate under a different structure completely.  No orders.
Some delegated acts that we do (as in giving immunizations).

Still this physician insisted that his patients not be referred to an LC
and that his practice had that expertise on staff.  Needless to say,
they do not and (at that time) the nearest LC was in another county
altogether.  

Things are changing since then.  Now the territoriality is directed to
NP's and  midwives.  Why it's okay to complain about have too few
resources and being overworked and then refuse to refer is beyond me.
