One alternative to using a nipple shield, which might bring about any
number of problems to a baby who is already having trouble latching, is
to use that great technique described as a flipple earlier -- never
heard that term, it's great! :-). At the same time, the baby could be
topped up with hand-expressed post-feed ebm in a cup. A cup can
encourage a baby to stick his tongue out a bit farther than usual, in
case that is contributing to his problems. Hand-expressing will help
keep up a milk supply that is probably vulnerable by now.

And the baby will develop his breastfeeding expertise, grow, and get a
bigger mouth... The trick is to keep him full and happy while we wait
for the mouth and the nipple to accomodate each other.
