We also had this dilemma at our hospital.  We were charting in the mother's and baby's chart.  At the end of last year, I felt that a lot of information was being lost in the paperwork quagmire and put forward a proposal for a 'lactation sheet'.  We then designed a specific sheet pertaining to lactation which nurses, peds, and us LC's can refer to.  It has lots of good info such as time/date of birth, method of delivery, anatomical details, whether or not mum took a class or has had prior breastfeeding experience, whether colostrum was expressed or not, whether mum followed suggested guidelines/breastfeeding plan verbally and by demonstration...and more.  It has lots of good and relevant info on it, if only the nurses and peds would read it.  I think we (LCs) are the only ones that really look at the info from the previous day, as I really think the postpartum nurses don't know how some of the details are relevant to lactation.  They just don't
 have enough breastfeeding education which I feel should be mandatory if you are going to ask for a postpartum or LDR placement.
