Oversupply:
I agree with Gonneke,
This is not an oversupply for twins at all. I also do not believe that O/S is the cause of recurrent plugged ducts or mastitis. I believe either inefficient emptying and/or inflammation in the mother's body is the cause. When I have a mom with recurrent plugged ducts, I first use homeopathy, magnesium and vit D. If I need to go further (or if I have an open mom), I look at her nutritional profile. 


Contraceptives:
What so troubles me about this Lisa is that is the exact same paternalistic argument that was used 15 years ago in my hospital. Ver batim!!!  The assumption is that if you are poor, you are stupid, uneducable, irresponsible and definitely should not have more children. I want to know how manipulating women and downright lying to them (I had moms told they would not be allowed to leave the hospital w their babies if they did not take the depo shot!!) empowers them to stand up to their boyfriends??!! I want to know how they get to decide what is more important to the life of that family? I worked in a clinic with poor women. Guess what? They understand LAM! I want to know how destroying breastfeeding, which except for birthing, is the most empowering experience a mother can have, is going to help these moms stand up to pressure from others? I want to know what is going to happen when these moms hate how they feel on the depo and do not come back at three months and they have lost breastfeeding as well? Pregnancy maybe? We were told that our moms were irresponsible and did not return for their 6 weeks check-ups so they had to give the shot in hospital. Really? Data please? Then again--I would not go back to see someone who treated me with such disdain either.

I know this is tough, Lisa. But women deserve always to be told the truth. I once went and got a package insert for a mom who could not get it from a nurse (they would often not give them to the moms). I was accused of practicing medicine w/o a license in an effort to intimidate me. Did not work. We told our moms that depo dumped supply when given in the first 6 weeks bc it does. I believe that we can never collude with the control of women by an institution. We have to tell the truth or we have no right to be their providers. 
As you say, life changes will not come quickly. They will come much sooner though if you stand for their rights and be the honest person they can rely upon. And if they make choices you do not like, or their doctors do not like, well, that is what freedom means, isn't it? At least every mom who will indeed make choices to prevent unwanted pregnancies will have had the opportunity to give good care to their babies and themselves (making them more likely to not want another baby soon) and those who are pregnant again will at least know that they are responsible for their lives and they can rely upon you for the truth. I think that is a win-win frankly.

I need some suggestions for  Mom with recurrent plugged ducts/mastitis and 
twins.  She is currently pumping for 20-30 minutes every four hours and 
producing over 50 oz per day.  She could pump longer, she is not at drops when 
she stops.  (The babies, recently released from the NICU are not yet at the 
breast, and are taking in less than that).

I want her to be more comfortable AND have enough milk for her cuties, but am 
unsure how this works with twins.  

Does anyone have a really great tried and true pumping protocol for a situation 
like this?  

We are trying a variation of "block feeding", in this case "block pumping" for 
the time being.  She is aware of lecithin, B-complex and Vit-C as nutritional 
support for recurrent plugged ducts.   She had been drinking Mothers Milk tea, 
and has agreed to stop taking it.  I am reluctant, with the twins still being so 
small, to use aggressive pharmacological means to drive the milk supply down, 
but am open to suggestions from those with more twin experience.

Changes to Guidelines for Contraceptive Use Could Compromise a Woman's
Ability to Breastfeed 

 

So I've just read this press release and it is stirring up memories of
recent discussions at work.  While it reflects my personal philosophy of
"let the mom choose," I have been chastised a few times by our public health
OB dept staff when I've complained of mother's milk supplies being killed
off by depo.  They have a different concern that they feel trumps
mine-mothers who cannot say "no" to the men in their lives, and who are not
reliable in taking pills or whose partners will not use condoms. They tell
stories of women being plied for sex by impatient dads when they are barely
out of the hospital (sometimes they are not even discharged yet!), women who
are down-trodden and unable to stand up for themselves. They believe that
protecting these women from unwanted pregnancies is more important than
breastfeeding.

 

I would be very interested in how others have dealt with this.  Never mind
LAM, we cannot go there.  How else can we reach a win-win situation for
mothers and babies? Life changes for these women are not going to come
quickly.
