I've done both sliding scale and barter.  When I do sliding scale, I
usually charge at least something, so that the mom values the time I
spend with her.  Usually I offer 50% of my usual fee, with the rest due
"when you can afford it" and then I write off the balance.  I very
rarely get that other half.  For people who can't afford that, I tell
them my usual fee, and then tell them they can come to my office and pay
me what they can afford at the end of the consult.  Sometimes I get more
than I would have asked for, and more than they told me they would be
willing to pay before the consult.
