Rose,
How does the baby look and feel? What is he doing? How does he
behave? Is he meeting developmental milestones?

He had quite a slowdown between 4 and 8 months in weight gain (was
tracking along 50th %ile then down to 15th), but not in length
(staying along 50th %ile). He could simply be an active baby who is
busy moving around, and is slimming down a bit from some fairly rapid
weight gain in the early months. He may just be settling into a size
and shape that are right for him.

It is possible that, being the fourth baby, he is not getting as much
of his mother's energy and attention as he would like, and the
breastfeeding is suffering. Or there may be issues around
introduction of solid foods that have affected his growth. These
would warrant breastfeeding assistance. But I would take a good look
at the baby before doing anything else. I think this pattern of
growth could easily be perfectly healthy and normal for this
particular baby.
