I use the definition outlined by Labbok & Krasovec; exclusive meaning no
additional fluid (medication, formula, water, vitamins, etc).  Therefore,
giving any formula for whatever reason would eliminate exclusivity.  

As far as maternal nipple cream, my own judgment call would be that
exclusivity would remain intact (the cream was not prescribed for oral use
in baby and it is not a liquid).
