My question in relation to including sucking on a dummy (or finger or
anything else) excluding infants from being exclusively breastfed is why
this is relevent?

The meaning of exclusively breastfeeding could be interpreted as no other
sucking than exclusively at the breast. Sucking a pacifier would not be
classified as exclusive, while also for one supplement no matter how small
with a bottle for two reasons: the artificial breast milk as well as the
mode of ingestion.

I may be being much narrower in definition than anyone here, but yes,
when a baby is given any medication my mouth, they are not 'exclusively
breastfed' during the time they are taking medication. Exclusive
breastfeeding means that the only things baby swallows are its own
saliva, mother's milk, and any tiny amounts of mother's blood, sweat,
etc. that are consumed as part of breastfeeding. What I'm not sure
about is if a baby whose mother is using nipple ointments can be called
exclusively breastfed.
