The problem with the term "under normal circumstances" is that it can be
widely interpretated.  Also, besides fluid loss, physicians are concerned
about low glucose levels related to infrequent or delay feeds.  Is the
infant of the diabetic mother a normal infant?  Is the infant of a medicated
birth a normal infant? We have to answer these questions too, or risk these
infants being included or excluded as "normal".  Auerbach mentions that  the
infant who has not nursed for 12 hours, and has not latched at birth, should
have appropriate interventions p. 221 which may include alternate feeding.
