I heard back from JHACO. They are very helpful. If you ever have a question about a core measure, you just register on their website and then send an email. They usually get back to me within a couple days. Their response is below.

QUESTION: 
Breastfeeding Core Measure  
    
For the Breastfeeding Core measure, what type of certification does the Lactation Consultant need to have? Does she need to be an IBCLC? 


I have a friend whose QI dept has told her that the Physician must chart, "Mother refuses to breastfeed." When the mother chooses exclusive formula feeding as her preferred feeding method. Is this wording required?

ANSWER: 
IBCLC or CLC (Certified Lactation Counselor is acceptable.


There is no Joint Commission requirement to chart the patient refused to breast feed. There needs to be documentation upon admission (birth) of the newborn in the newborn's medical record of the initial feeding plan discussion and what the mother decided to do.
