How unfortunate for this mom to be in such severe pain.  I see you are in
Austin, if possible could the mom be referred to Barbara Wilson-Clay, an
outstanding lactation consultant who is a lactnet participant. Not sure if
she is taking referrals, but if so, that is my first suggestion.

Also check to see if the baby has any anatomical variations, esp. tongue-tie
(ankyloglossia). Have the mother express regularly with the method that she
chooses which is comfortable to her. This may be a hospital grade pump with
a large size flange, or a manual pump, or hand expressing.
You mentioned the baby pulls his chin up, maybe he is compensating or maybe
even cranial sacral therapy could help him. Also the possibilty of emotional
and/or psychological issues could be superimposed on any of this.
