Hi Mary

We start with hand expression for the first 24 hours and almost full-time
skin-to-skin.  We hand express 2 hourly and I don't find the process too
time consuming.  We don't remove the baby while expressing.  If the mother
has an unusually large amount of colostrum we may start pumping later on day
one if it looks like baby is not going to latch anytime soon.  Usually we
only use the pump on day two.  We combine hand expression with pumping.  I
have not noticed a problem with oedema.

Our babies are not routinely suctioned, but many are disorganised after
birth (usually C-sec or vacuum extraction).  Have just discharged 3 premies
(32 weeks) who were skin-to-skin from birth and went home on day 4 fully
breastfeeding!  I am truly grateful for wonderfully supportive
paediatricians!
