I would try a newborn (not small) nipple shield in this situation.  Baby
may have become dependant on the firm stimulus of the finger in the roof
of the mouth to initiate suckling...or may have had a dysfunctional suck
all along, which led to extreme weight loss.  Be sure baby does not stop
where the tip of the shield meets the flat part, but continues onto the
breast itself.  Hopefully won't need to use it for more than a few
days/weeks.
