Cheryl, I am sorry that I have not responded sooner to your question
regarding what treatment my patient with the history of pituitary tumor was
treated.  Completely unrelated to this patients situation but to explain why
I am just now responding... my own daughter ( she is 11 years old ) was
diagnosed with a tumor on her pituitary on Friday afternoon (results of an
MRI) A coworker was able to come see my patient and I have been running the
doctor circuit getting labs done and evaluations....

Anyway, I saw this patient today, She is 29 years old. Her tumor was
diagnosed when she was 19 years old and living in Idaho.  She was treated
with Parlodel but she said that she did not take it for very long... maybe 6
months not sure... because it made her feel too bad.  She said it made her
feel completely wiped out, gave her headaches made her miserable.  She quit
taking it and did not get any further treatment or follow-up.  She had no
problem getting pregnant and her breast size increased well during her
pregnancy.  She delivered on 9/14 and feels her milk came in on Sunday 9/19.

 On Thursday 9/16, she started using the nipple shield, and pumping after
every feeding.  Baby weighed 8# 8 oz at birth, on 9/17 at pediatrician baby
weighed 7# 13 oz.  On Monday 9/20 baby weighed 8#8 oz ( again at
pediatrician)  Mom was using some formula after nursing over the weekend
because the baby seemed hungry and she was getting nothing when she pumped.
On Sunday she was able to get better results and quit using formula
completely.
Today, baby weighed 8# 7.8 oz on my office scale. Nice abundant yellow seedy
stools, soaking wet diapers, well hydrated, content, alert baby( To be
honest, I trust my scale more, the ped office she goes to uses a scale that
does not measure increments of an ounce and uses a sliding weight marker) On
today's test weight the baby transferred 3.4 ounces of milk using the shield
in less that 40 minutes.  Mom felt much softer after the feeding and was
very pleased.  Baby was very pleased as well!!!  He nurses about 10 times in
24 hours and seems to be doing very well. He is giving her a 3 or 4 hour
break in the wee hours of the morning, otherwise his feedings are every 2-2
1/2 hours during the day/ evening.  We are having a follow up visit on
Friday to make sure he really is gaining based and that the other
measurement really is just a difference in scales.

So it seems, so far, that there was a delay in adequate supply but that
supply is now achieved.  Whether the delay is related to mom's medical
history, or because the baby was not adequately nursing to stimulate
production or a combination of both I do not know. I am happy with how
things are going and keeping my fingers crossed that they continue to go
well.  :)
