Recently I began working inpatient as a hospital based LC.  What is 
needed is a new a standardized flow sheet that goes on Mother or Baby
chart for inpatient lactation visits.  My previous employer had LC's chart 
on mother and baby's chart after each lactation visit.  Plain and simple.
Do you have any suggestions on where I can look into finding a great and
streamlined form.  The current hospital's form is way too long and takes up
a great deal of time  filing out and it is duplicating what is already on 
the mother or baby's chart.
