It has to be dispensed in a unopened bottle otherwise you would run into issues with contamination.  In our hospital any infant who is bottle feeding is given 1 unopened carton (6-8 2/3oz bottles) to start with.  It is placed in their crib upon admission.  Breastfeeding infants are not given formula unless it is medically indicated or by maternal request after exclusivity teaching is done and documented.  Then they are given 1 bottle at a time and taught how to cup feed using a small medication cup (we've been trying to get foley cups..it's a work in progress, I digress), starter SNS with help or a nipple if they insist.   

And yes there is huge waste.
