Dear Friends:
     AAARGH!
     Mom with 3rd baby. BF the first 2 for 3 months each, until she went back
to work.
     3rd baby found for be 1/2 pound under birth weight at 14 day check up.
MD suggested a LC consult. Good.
     I see mom. Complicated case (aren't they all?).
     1st, she had a spell of heavy bright red bleeding and passed several
silver dollar sized clots 4 days before I saw her. (day 14)  Possible impact
on supply there. She has noticed an increase in leaking since passing the
clots.
     2nd: she has had implants on both sides after baby #2. Is complaining of
aches and pains in both breasts that she hasn't had before. Consequently,
these are brand new breasts for her and I wonder if her storage capacity has
been diminished because she doesn't remember the 6 and 8 year old children
nursing as often as this baby wants to.
     3rd:  her mother is visiting, to help. This is good. However, grandma
feels her job is to soothe the baby every time he cries. Possibility of
missing feeds  because grandma is so adept at soothing baby.
     4th: this mother likes to use a pacifier. Baby did great at night, only
fed twice, at 3-hour intervals. Turns out she uses the pacifier a lot at
night. Another reason to miss feeds.
     5th: latch is shallow. Consequently, nipples are sore.  Mom never had to
support breast underneath before. She doesn't like it. Will try rolled up
washcloth.
     6th: mom is too busy to pump more than 2-4 times a day. She did get a
hospital grade pump. She doesn't like using it.
     6th: After my visit, and dealing with all these threads, mom isn't sure
that breastfeeding is for her. "This baby wants to be at breast so
much....can't she just give a pacifier? " I remind her that the baby is
nursing a lot to catch up on weight, and that this is normal for a newborn
baby. She isn't sure....things were so much easier before.
     But her baby wasn't gaining weight!
     AAAARGH!!
