It doesn't happen often, but it has happened occasionally, like last week,
that a doctor is willing to perform his first frenulotomy if an IBCLC is
there during the procedure.  If this sounds scary, remember, doctors are
trained to do minor surgery, and every doctor who routinely does
frenulotomies had a first.
