I've worked with a Mom with a similar scenario - IGT, previous FTT children, extremely well informed, willing to do anything to keep baby at the breast, and baby nursing ALL day and night.  After trying many interventions, we found Mom made more milk and baby gained more when we discouraged 24 hour non-stop nursing and added in specific times and amounts to supplement.  While Mom didn't want to supplement at all, I think she experienced less anxiety overall because it took the guess work out of supplementation.  Previously she would try to evaluate how the baby was feeding throughout the day and supplement in small amounts in response to her assessment (and that takes a lot of the fun out of nursing).  When baby got a larger supplement, he slept a bit, Mom had some downtime and some time to pump a bit and then baby woke to be more effective at the breast.  

This Mom spent much time deliberating before trying this plan but decided to go ahead with it as a trial and see what happened.  This was her 4th and by far did the best of all children gaining while nursing with the least amount of total supplement.  

Not sure that this is the right answer for your Mom, but something to consider.
