As soon as you mentioned that all her babies were slow gainers, my first thought was that it's tongue tie, and this is just the first baby who can't compensate enough to stay on the charts.  If you can get a look at the other kids and convey the idea to mom, she may have a lightbulb moment and be pretty forceful about getting it clipped.
