I am working with a new Mom (permission to post), first birth, identical twins, born by c/s at 38 weeks due to both being breech. This pregnancy was after 3 miscarriages and a history of high prolactin. She took one half comed and one shot of Oatral. She was on Pregnal for the first seven weeks 2 time a week. She has mild hypoplasia. 

She breastfed the babies at about 8 hours after birth and due to a total lack of encouragement from hospital staff (including the ibclc- according to what she said) she mostly bottle fed and did not do much pumping. 

I saw her 11 days postpartum and she is curently able to pump between 30-35 cc milk, according to her account. I saw both of the twins (who by the way are tongur tied) latch well and saw quite alot of continuous swallowing. They are small but seem quite strong and alert. She has no pain when she nurses.

I suggested that she check her current prolactin levels and pump for stimulation as well as breastfeed before offering the bottle. I felt that motilium would be appropriate and advised her to discuss this with her doctor.

My questions are: 
Are there any other tests she should do?
What should I be looking for in the prolactin level test?
How much can motilium help in cases of hypoplasia and hormone imbalance?
She is interested in using an SNS - would that be appropriate in such a case?
Am I being over optimistic that she can give at least partial breastfeeding?

I am trying to keep her optimistic and spend as much time sts with the tiny girls and she seems much happier that someone is believing in what she thought should be true.
