If memory serves me correctly, it is important whether prolactin levels
are drawn when mom is fasting, or how many hours after a meal, whether in
the morning or afternoon or night, before nursing, after nursing, etc. In
other words, expensive as it may be, serial prolactin levels under
various of the above conditions, would seem to give a better idea of her
aggregate "prolactin level", because a single level, without
consideration of the above factors, makes a difference. Perhaps someone
with a reference handy can comment further.

BTW, my client with the (formerly) clamping baby and insufficient
glandular tissue, after 5+ weeks of frequent nursing, some supplementing
with tube at breast, insurance pumping after nearly every feeding,
fenugreek and mother's milk tea,  is now allowing the baby to sleep for 4
hours at night, but begins to feel "uncomfortably full" enough to have to
pump, (which seems counterproductive to let the baby sleep, to me,
because the mom still isn't getting to sleep the 4 straight hours. [I
myself have not been there to observe, of course.]) But subjectively, her
perception, "feeling so full she must pump" is obviously pleasing to her.
(This was occurring before the domperidone she started 5 days ago, and
she has noticed no additional change so far.)

Her latest question is about going out in public-she has no problem with
nursing, but worries what will happen if she doesn't pump right after
that feeding. (getting 0.5-1 ounce after each feeding now, and baby is
also taking just a few ounces of formula supplement 1-2x daily.) Baby is
no longer being so closely weighed as in the beginning, but is
"outgrowing his newborn sleepers".

However, her original issue, traumatic memories from the first child, of
terribly painful nipples, was completely resolved by time and CST, and
she has had no nipple pain at all for weeks.
