I would probably try as best as possible to show this mom different  
positions wherein the baby could self-attach, like side-lying (there  
is a great video in the new BF Atlas of Barbara Wilson-Clay teaching  
a mom how to let her baby self-attach in this position), or having  
mom reclining and baby upright on her chest/abdomen.
