Duvets (which are big down comforters, more or less) are one of the specific
risk factors that Peter Fleming discusses.  He also discusses the dangers
associated with overheating.  I interpret this to mean one should dress baby
warmly enough so you can keep the room on the cool to temperate side without
using big covers that could drift up around the baby's face.

As to pillows and linens, one should keep them to a sensible minimum.  I use
so many pillows reading in bed, etc.  that I would have to change my habit
in order to safely co-sleep with an infant today.

I like to mention the sidecar arrangements including commercially made
sidecars as well as normal crib pushed up against the parental bed as
alternatives too.
