Lisa, EXACTLY!  Very well  said.  The important thing for us as IBCLCs is to 
know how to advise  mothers on taking medications.  For example, my daughter 
takes  synthroid.  Synthroid MUST be taken on an empty stomach as taken with 
any  other drug -- or food -- will decrease its potency.  Interestingly enough,  
thyroid meds are often given in the hospital during routine med rounds --  
9-1-5-9 or 10-2-6-10 -- they pay no attention to that whatsoever.  (Just  
checked on that with my dh, the pharmacist, and sure enough, at the hospital  where 
he is director of pharmacy, it is given as a routine drug, once a day, in  the 
morning with all the other meds).
 
However, when Torrey started  taking hers on a empty stomach as FINALLY told 
to her by her endocrinologist  AFTER she had been on the medication for about 
8 years, she could decrease the  dosage because it was more effective.
 
So the point is, we need to  know what is going to be effective and how 
should people take it.  If it is  going to decrease the effectiveness of other 
drugs (i.e. St John's Wort  decreases the effectiveness of oral contraceptives -- 
so you may have a lot of  very happy pregnant people), we need to know what 
those are so we can advise the  mom appropriately -- not just give a blanket 
statement of because this can  happen, we can't recommend that.
