I don't know which countires you consider developed, but there is a document 
with rates for Europe, which was used to inform the European Blueprint for 
Breastfeeding.  Go to :

At the bottom of the page are links to documents, download 'Current 
Situation' and on page 14 you will find some rates.  See where the UK is -- 
way down near the bottom, heigh ho.  The blueprint is also downloadable at 
this site. 

Hope this some help, doesn't probably cover all you want, though, I'd guess.
