You missed it.  We use an "Information Sheet" not a consent form for
Pasteurized donor human milk- it is there, following the P & P re use of PDHM.

If parents wish to use pasteurized donor milk, they can.  We don't offer it
routinely, but it is available for even full term infants who need
supplementation.  I would love to use it for ALL supplementation, but as most of our
patients are on a fixed fee for the hospital stay, we would "eat the cost" so to
speak.
