Dr. Coryllos does them for children of all ages, as long as the child is 
able to hold still or be held still.
She has done simple frenotomies on  5 and 6 year olds with their 
cooperation in the office (faculty practice building next door to the 
hospital, set up for minor surgeries). She tells the story of one little 
guy who told her "I'd be able to talk better if they just cut this 
string under my tongue!" and then held still for her while she did it. 
She uses flavored benzocaine gel or oil of clove to numb the frenulum 
before snipping it.
Things get more difficult when there is a blood vessel in the frenulum, 
then suturing the vessel and cutting between the sutures might be 
necessary, and so might cauterization. Some doctors use a laser, which 
might help to reduce bleeding too.
