I agree with you that infants with high risk of morbidity and mortality from
NOT breastfeeding (such as your population in Zimbabwe) should continue to
nurse with HIV if the Mom chooses.  I think WHO/UNICEF were crazy to change
their prior recommendations.  But in the USA and other developed countries
the situation is very different.  I would have offered the Moms in question
the option of heat-treating their own milk, or using donor milk.
