The Breastfeeding Atlas has good photos of this condition, and discussion of
how and why it causes trouble with breastfeeding.  Mine is at work so I
can't give you page numbers.  There was an oustanding article in
Contemporary Pediatrics Vol 21, No 5.  The tilted mandible is caused in
utero by pressure from baby's shoulder against chin.  (Lean your head to one
side and push your shoulder against your chin and you'll feel your mandible
moving out of alignment.)  In severe cases the side against which shoulder
pressed will be very evidently indented, ear elevated and eye on that side
smaller and higher.  + torticollis of course.  My experience is baby will
look better with time (and exercises/PT) but tilted mandible persists.  I
have found this tilt and offset to be a problem only in severe cases.
