I am an RN in Paducah, KY getting my lactation observation hours to get my 
certification. I am currently volunteering with the lactation consultant at the 
hospital I work for. We are also wanting some information about the same 
things you are asking for about fees & charges. It does not look like anyone 
ever responded. Did you ever find out any info on this topic? Please contact
